The Houston Center for Independent Living has resources available for patients in nursing homes who are medicaid or medicaid eligible to help them transition back into the community, if it is possible. Contact them to have an assessment done and to identify resources available. The great part of this program is that if you qualify you can access community based services as a priority and not have to be placed on a waitlist. There are also some funds associated with this program to help with home modification, equipment, etc.
